A Patient-Sponsored Medical Home Model Practice
Our practice focuses on providing high quality and well-coordinated care, with an emphasis on providing a higher level of individualized attention. The design of our medical practice has been influenced by two primary care models. It is a hybrid of a “Patient-Centered Medical Home” (PCMH) model practice and a retainer model practice. Retainer model practices provide medical care to a smaller panel of patients, allowing their patients to purchase amenities that supplement traditional primary care, including more personalized service.
The “Patient-Centered Medical Home” has been endorsed by many organizations, including the American College of Physicians and the American Medical Association, as a model for high quality primary care medical practices of the future. Our practice model will aim to incorporate many features of the Patient-Centered Medical Home with criteria as defined by the National Committee for Quality Assurance.

Annual Comprehensive Health Assessment
An annual comprehensive health assessment allows us to review your medical history, family history, lifestyle factors and physical exam findings that contribute to your health risk and make recommendations for health promotion and the prevention of future illness. At this visit we will review current evidence and preventive guidelines as they relate to you personally and make recommendations.
Most insurance plans include an annual preventive physical exam. If we participate with your insurance plan, your physical and standard physical exam lab work will be billed to insurance. However, our practice model allows us to take extra time with you during this visit and improves our ability to be very complete in our assessment of your health risk. In addition, an extended laboratory preventive screening panel and any elements of our extensive annual health assessment that are not covered by your health insurance will be provided as part of your amenity fee.
Standard Physical Exam Lab Work

- Pap smear with automated high risk HPV testing
- EKG (baseline at 40 then as guided by health risk)

- Hs-CRP test (test of "inflammation")
- Expanded lipid panel screen (lipoprotein particle size and density measurement)
- Hemoglobin A1C (three month average blood sugar equivalent)

- Colonoscopy
- Pelvic ultrasound
- Hepatitis screening
- Sexually transmitted infection screening
- Immunizations as per guidelines
- Clinical genetics referral for genetic testing

- Coronary calcium scoring
- Abdominal aortic aneurysm screening
- Health risk assessment screening for sleep apnea, breast cancer, coronary artery disease, depression, osteoporosis fracture risk, fall risk and screening for cognitive impairment.
Hospital Care and Affiliations
Personalized Primary Care Atlanta is a private medical practice. However, for our patients who require inpatient care, Dr. Tong and Dr. Mavromatis will admit patients to be cared for by the Hospital Medicine specialists at Emory University Hospital or Saint Joseph's Hospital of Atlanta.
For those who receive care in other local hospitals within the Atlanta perimeter, such as Piedmont Hospital, Dekalb Medical Center, Northside Hospital, Emory Midtown, or Atlanta Medical Center, Dr. Tong and Dr. Mavromatis are available for courtesy visits and to help with coordination of care and transition planning.
Our practice is part of the Emory Health Network. We have access to Emory's electronic medical record.
