Health Insurance

For office visits, our practice contracts with many health insurance plans and submits claims for you. If you pay for your medical care directly, or if your health insurance does not contract with our practice, our office fee schedule is reasonable and will be accessible on request. We will attempt to refer to preferred providers for specialty consultation, diagnostic testing, and inpatient care in order to minimize your out-of-pocket expense. If your health insurance does not participate with our practice, we have options for membership that will not result in significant increased out-of-pocket expense for you beyond your annual membership fee.

Our practice will be happy to help you navigate your health insurance and will listen to your concerns as health insurance policy evolves in years to come.

Annual Fee

PPC Atlanta is committed to delivering high quality healthcare services to each and every patient.   We will limit our practice size to 500 patients per physician (compared to standard practice sizes of 3,000 patients per physician). As a patient within our practice you will receive service amenities above and beyond what is covered by health insurance and managed care programs. Our annual fee covers the cost of these amenities. Annual and quarterly payment options are available.

Individual

Annual: $1,950 per year, per member

Quarterly*: $530 per quarter, per member 

Family (adult couple with dependent children)

Annual: $3,500 per year per family

Quarterly*: $960 per quarter

Dependent Children: We will include dependent children between 16 and 26 years old in your family membership.

* 3 month minimum excluding Annual Comprehensive Health Assessment, or 6 month minimum including Annual Comprehensive Health Assessment

First Time Patients

Dr. Tong and Dr. Mavromatis  are available for a complimentary Meet and Greet Visit. Contact us if you would like to schedule a Meet and Greet Visit.

Medical Spending Accounts/Health Savings Accounts

Our practice fees may be eligible for reimbursement through your Medical Spending Account or Health Savings Account. See IRS publication 502, “Medical and Dental Expenses.” We will provide an itemized receipt for you annual fee to submit to your HSA/FSA provider.
